simple, what you have to do is just get lot of referral and be premium member, and be active clicker. Just that... or also you can join the other paid to click site.

well if you have lot of money you can buy a referral, but im not trying to ask you to do that, because there is still lot of way to get referral. you can try referral program to get free referrals.
how can I get cashout?
of course you need some payment processor, like e-gold and alertpay. They will pay you with those payment processor.
are they really gonna pay?
Depend what site do you join, be aware for some site that offering big cash with really easy way. Just read it carefully and don't forget to read the TOS, and also try to check their domain and server, but if you don't understand how to check it, well I could really help you for that.
